Tracking California’s Sickle Cell Data Collection for California program invites you to attend a webinar, “Stepping Up into Adult Care: The Sickle Cell Trevor Thompson Transition (ST3P-UP) Project. This is the third in a SCDC mini-series on adolescent transition from pediatric to adult health care.
Principal Investigators Dr. Ifeyinwa (Ify) Osunkwo, Carolinas HealthCare System, and Dr. Raymona Lawrence, Georgia Southern University, will present.
This multi-center study will compare the effectiveness of adding virtual peer mentoring to a structured education-based transition program for emerging adults with sickle cell disease. The outcomes studied will be decreasing the number of acute care visits per year, improving patient-reported outcomes, and reducing healthcare utilization among emerging adults with sickle cell disease.
The agenda will include a brief update on the Sickle Cell Data Collection program (the host of the webinar). This webinar is part of a series of quarterly webinars featuring experts in sickle cell disease and updates on this project. This work is funded by the CDC Foundation, Pfizer Inc., and Bioverativ Inc., and is a partnership with CDC’s Division of Blood Disorders.
